Access to Care received a sub-grant award to collaborate with Metropolitan Family Services’ Portage Park and Norwood Park Health Equity Coalition, a hyper local strategy, part of Protect Chicago Northwest region initiative, led by the Northwest Center. This coalition aims to decrease health inequities by maximizing outreach, collecting residents’ input, and leveraging hyper-local resources in the Portage Park and Norwood Park neighborhoods of Chicago. These communities continue to be deeply impacted by COVID-19 and require continued support in 2022. Access to Care is a non-profit primary health care program for low income, uninsured, and underinsured residents of suburban Cook County. Access to Care member services include physician office visits, laboratory and radiology, prescriptions, and mental health. Members utilize services at locations close to their homes, and in doing so, a local network of primary care services in the patient’s own neighborhood is created.
