Cook County Health is embarking on their 2023-2026 strategic planning process and wants to hear from the community. The strategic plan will define and guide CCH’s goals for the next three years and elevate their historic mission to care for all. Cook County Health is asking their valued partners and stakeholders to complete a survey to share their perspective on CCH’s strengths, weaknesses, threats, and opportunities.
