The Cook County Clerk’s Office will host a Back-to-School Birth Certificate Event on Saturday, July 20th, at Chicago and suburban locations for families seeking to obtain birth certificates to complete their child’s registration for school. To accommodate working families who may not be able to visit the Clerk’s Office of Vital Records on weekdays, the office will be open at locations in Chicago, Markham, Maywood, and Skokie on Saturday, July 20, from 9 a.m. to 3 p.m. The following is required to get a certified copy of a child’s birth certificate:

• The child must have been born in suburban Cook County or Chicago;

• The attending adult must be listed as the child’s parent on the birth certificate or be their legal guardian;

• The attending adult must have a valid photo ID, such as a driver’s license, federal or state ID card, military ID, U.S. passport, and/or supporting documents;

• A $15 fee is required ($4 for each additional copy)
