Esperanza Health Centers Launches Panorama PACE on Chicago’s Southwest Side

Esperanza Health Centers is eagerly announcing the launch of Panorama, a Program of All-Inclusive Care for the Elderly (PACE) that delivers comprehensive, community-based care for seniors while providing a needed alternative to nursing home care. Located at 4720 S. California Avenue in the Brighton Park neighborhood, Esperanza’s Panorama PACE center is officially open as of July 2024 and offers a wide array of services that enable older adults to live independently and remain in the communities they know and love. To be eligible for Panorama, participants must be 55 or older, be certified by the state as needing nursing home care, be able to live safely in the community at the time of enrollment and live within the designated Panorama service area zip codes. The center will be open Monday through Friday from 9a.m. to 3p.m., providing a “home away from home” for seniors with state-of-the-art facilities, including primary and specialty care exam rooms, behavioral health counseling rooms, dining room, activity areas, and other indoor and outdoor amenities. To familiarize the community with Esperanza’s Panorama PACE facility and the all-inclusive services on offer for older adults, Esperanza will be hosting an open house on Thursday, August 8th from 11:30 a.m. to 4 p.m. Community members will have an opportunity to tour the facility, speak to Panorama staff members and learn firsthand of the benefits that PACE can bring to older adults and their families in Esperanza’s service area.
